Health Service (ISCHS)
Inner South Community Health Service is a major provider of health and community services in the inner southern region of Melbourne, Australia. Our vision is of a healthy and inclusive community.
We work towards this by:
- Delivering primary health care services across four sites. These services span allied health, dental, drug and alcohol, mental health, casework/counselling, family violence, and sexual and reproductive health.
- Developing links with GPs, local hospitals and other service partners, in order to integrate and coordinate care for people living with chronic conditions and reduce preventable hospital admissions.
- Pioneering new ways to improve the health and wellbeing of people and groups who are disengaged from mainstream services. We deliver a number of peer-based programs and outreach models to people with high and complex needs.
- Undertaking projects and programs that build the capacity of individuals and groups to improve their health literacy, take charge of their health, develop community connections and be agents of change in their local area.
- Undertaking systems advocacy (including social research, policy analysis and debate, lobbying and media) in order to reduce health inequalities.
We work across the life span and have a strong focus on prevention, early intervention and addressing social and health inequity.
